What I'm presenting here is a first draft / idea set, put up here for feedback.

The basic notion is that, past 20 hit dice, instead of gaining more levels, you start to convert existing levels into gestalt levels. Of course, there need to be a few modifications...

1) You get full skill points for whatever level you add. (Otherwise, for example, a 20th level rogue would never gain skill points again...)
2) Skill max ranks increases by one per two levels past 20. So 24 at level 22, 25 at level 24, and so on.
3) Cross-class skills no longer cost double.
4) You get one feat every two levels, starting at level 21.
5) Epic level feats are available with GM permission - and may be available even if you don't have the listed pre-requisites (especially in the case of things that were written assuming you gain max skill ranks at one per level...)

6) Inherent equipment needs to progress at some rate. Maybe 20-40k per level?
7) XP costs per level can't continue their astronomical climb, since actual power isn't increasing that much.

It's somewhat unfriendly for more-or-less straight class characters... Gestalt Psion/Psion?

That's somewhat deliberate - though, perhaps, overdone.

I don't really want people pushing through to ever-higher caster levels and spell power. Some advancement is reasonable - maybe one caster level per three or four levels past 20? Or go find one of those prestige classes that specifically gives bonus caster level (archmage or hierophant for normal casters; presumably there's a version floating around that'd work for a psion?)

The notion is that a 30th level character doesn't have boatloads more raw power than a 20th level character; he just has more flexibility.

Version two:

For each of base attack and the three saves, calculate as normal using your 20 best levels, whatever those may be. Do this separately for each one - a fighter 20 / wizard 20 will have a base attack of 20, a base fort save of 12, a base reflex save of 6, and a base will save of 12.

For hit points, calculate as normal - but your con bonus (or penalty) only applies to the first 20 hit dice.

For skill points, calculate as normal. However, there are two changes to how you can spend them:
1: Any skill that has ever been in class for you, is still in class now.
2: Skill max ranks increases at one per two levels past 20, starting at 22.

Feats: One per two levels past 20, starting at 21.

Statadds: As normal - so your next statadd would be at level 24.

Inherent Equipment: 25k per level - just enough for a +1 inherent bonus to a stat...

Spellcasting and special class abilities cap at effective level 20. So, for example, a rogue 20 / assassin 10 would have 10d6 sneak attack, not 15d6. This is probably the fuzziest rule - talk to me if you have specific questions or concerns. Note that this example assumes the character did not spend any feat slots on more sneak attack.
